Medical Home@Work™. We’ve trademarked it, you’ve seen it on our marketing material, and now we’re dedicating a blog post to explain it. So what exactly is it? Why are we so big on the concept of Medical Home@Work™?

Quite simply, Medical Home@Work™ is the essence of what we do at Marathon Health.

The medical home concept is an approach to comprehensive primary care that is based on a partnership between the patient and the caregiver.

The National Committee for Quality Assurance, a private, not-for-profit organization dedicated to improving health care quality, describes a medical home this way:

“The Patient Centered Medical Home is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.”

That’s a pretty good, big-picture description of the medical home model.

But we take it one convenient step further. We added the “@Work™” to Medical Home because our model is available at your place of employment.

Our Medical Home@Work™, which is supported by our HIPAA-compliant Health Engagement System™ technology platform, begins once you meet with one of our clinicians at your onsite clinic. Our system includes evidence-based health programming and creates a Personal Health Record that serves as a data repository for your health information.

The components of our medical home include:

  • Preventative and primary care services
  • Referral management and tracking
  • Care coordination with your primary care physician, or our clinicians will act as your PCP
  • Community provider and health services integration
  • Patient education
  • Monitoring your condition
  • Follow-up care
  • Care gap analysis

The integration of community health resources is an important part of our Medical Home@Work™ model. The services integration is HIPAA-compliant so medical records are kept private and confidential. Community health resource integration includes, among other things, physical therapy, chiropractic services, radiology, occupational health services, vision, or dental.
